These Terms and Conditions apply to all services provided by Medical 2 Drive, including but not limited to driver medical assessments for licensing and employment.

3. Types of Medicals We Provide
Taxi Driver Medicals
Required for licensing by local councils.
What to Bring:
- Valid photo ID (passport or driving licence)
- Current optical prescription and corrective eyewear
- Medical form provided by your local council
- List of medications and health conditions
- Relevant hospital/GP letters if applicable
- National Insurance number
Note: Confirm with your council if it must be done by your registered GP.
Bus Driver Medicals (PCV)
For initial and renewal Group 2 licences.
What to Bring:
- DVLA D4 Medical Form
- Driving licence/passport
- Optical prescription and glasses/lenses
- GP details and medical history
HGV Driver Medicals (LGV)
Mandatory for new and renewing HGV drivers.
What to Bring:
- DVLA D4 form
- Photographic ID
- Glasses/lenses and eye prescription
- Medication list and GP/specialist letters
Motorhome Driver Medicals (C1 Entitlement)
For motorhomes over 3.5 tonnes.
What to Bring:
- DVLA D4 Form
- Driving licence and ID
- Prescription lenses and eye test results
- GP/specialist letters for chronic conditions
Ambulance Driver Medicals
For NHS/private ambulance roles or C1 renewals.
What to Bring:
- DVLA D4 Form (if applicable)
- Driving licence and photo ID
- Optical prescription
- Medical records or specialist letters
- Employer letter (if relevant)
4. Appointments and Contract Formation
How bookings are confirmed/rejected and reference numbers assigned.
5. Your Responsibilities
You must ensure correct medical type booking and bring all required documents.
6. During the Assessment
Vision and BP tests per DVLA standards. Retests may be free under certain conditions.
7. Cancellation, Refund & Rescheduling
Refund policy based on notice time, missed appointments, and cancellation terms.
8. Form Accuracy & Licensing
Check your form before submission. DVLA/council approval is not guaranteed.
9. Delays Outside Our Control
External delays may be refunded but we’re not liable for indirect losses.
10. Fees & Payment
All fees include VAT unless noted. Full payment required for confirmation.
11. Promotional Offers
Promo codes are subject to terms and can be withdrawn at any time.

13. Limitation of Liability
We are not liable for licence refusals or indirect losses. Maximum refund is appointment cost.
